- Go to the specific URL written on your AutoZone receipt or open the AutoZone Customer Satisfaction Survey site at www.autozonecares.com
- Select the country where you shopped between ‘USA and Puerto Rico’ and ‘Mexico’. If you choose Mexico, select the location from the drop-down list.
- Click the “GO” button and select a language to take your AutoZone survey in.
- Enter the six-digit Transaction Number shown as STR. TRANS. and four-digit Store Number, and Date in the empty boxes provided for them in the middle of the page.
